「なぜこんなところにshouldがあるんだ？」というのが、昨日の宿題でした。この文を解説するのに、まずは３つに分けるとわかりやすいと思います。

(1) The Kanagawa prefectural government is preparing a system

この部分は分かりやすいと思います。「神奈川県は制度を準備している」

(2) under which 6,000-9,000 consultation slots per day will be available

この部分を書き換えると、6,000-9,000 consultation slots per day will be available under a system

「制度のもと１日に6000-9000の診察の枠ができる予定」

(3) should fever outpatient clinics be unable to cope with a surge in patients

ジーニアス和英辞典の第６版の1880ページに、b)倒置構文でも用いられる：We would be delighted to hear from you, should you have any comments. 何かコメントがございましたら、お聞かせください(=..., if you should have any comments.). と書いてあります。これを参考にすると、(3

)は「万が一発熱外来の診療所が患者の急増に対処できない場合」と解釈できます。

元々の文が、下記のように書かれていれば、もう少し分かりやすかったと思います。
